How to prepare to get the most out of your visit with your musculoskeletal pain physicial

Once you have made an appointment with us at the Conservative Spine & Joint Center there are several things you can do to get the most out of your time with the physician. 


First, our staff always makes sure all new patients have access to the New Patient Forms. Our forms are available online at, or you can request our staff to mail them or email them to you.  Once you receive these forms, be sure to fill them out entirely, prior to your appointment.  By filling all paper work out before your appointment, you will ensure that you will be able to spend your entire visit consulting with the doctor.

For patients that are seeking a second opinion or have already seen another physician for their problem, you must request a copy of your medical record and/or any tests previously done. IF you have any test results like x-rays, MRIs and CT-scans, these are very important to bring with you to your first visit. These can be requested either from the doctor that ordered them or the facility where you had them done.  By signing a release of medical information you can request for these results to be faxed to our office so they will be here for your appointment.  

Keeping a log of your pain symptoms and a day to day pain scale for the last five to six days is a helpful way to get a more concrete sense of your pain.

The day of your appointment, be sure to arrive early with your insurance card, a current photo ID and all your forms.  It is also very helpful if you bring a written list of all the medications you currently take.

Finally, if you have questions for the physician, it is helpful to write them down so you do not forget to ask them during your appointment.
